COHME’s Dementia Support Programs are designed to guide, support, and empower both individuals living with dementia and the people who care for them. Whether you need hands-on coaching, emotional support, or ways to reconnect with your loved one, we’ll walk beside you every step of the way
What We offer

How The Program Works
Step 1: Comprehensive In-Home Assessment
We start by getting to know you and your loved one. We evaluate strengths, challenges, routines, and the home environment to create a personalized plan.
Step 2: Weekly In-Home Support
You’ll receive 1.5-hour sessions with a dementia specialist focused on:

Step 3: Ongoing Coaching & Resources

COHME is a licensed nonprofit home care agency serving New York City since 1984, with extensive experience supporting individuals with Alzheimer’s disease and related dementias.
The program is led by Leslie Mantrone, LMSW, who has rejoined COHME as our Dementia Specialist. Leslie brings advanced training in dementia care and our regular social worker Jamie Meconi.
Our interdisciplinary team includes nurses, social workers, and caregiver support specialists, allowing us to provide coordinated, psychosocially informed care.
We welcome collaboration and referrals and would be happy to discuss how this program can support your clients and care plans.
