Our Home Health Aides

COHME’s Home Health Aide staff are all trained and certified to provide the highest quality of home care. In addition to yearly required education and quarterly training, they receive ongoing supervision from registered nurses to provide care for those with dementia, palliative care needs and other issues.

Our Registered Nurses

COHME’s registered nurses are trained to make physical and cognitive diagnoses that shape your loved one's care plan. They make home visits every three months to monitor the client/aide relationship and approve ongoing care procedures.

 

Nursing staff are available 24/7 on call to ensure access to medical guidance and supervision to the aides. For an extra fee, our nurses are also available to do medication prepours.

Social Worker

Our Social Workers

COHME proudly honors our founding mission with one of our more unique services: an in-house Social Worker who oversees every case. 

 

Our Social Worker’s job is to ensure that you have the support, information, and referrals you need for healthy, ongoing care. Whether you need assistance with a living situation or an aide-related issue, our social work staff is available and ready to help.

 

As your special point-person at COHME, our Social Worker is a resource to you—helping you better understand and handle your loved one’s disability. They are an integral part of our approach.

Our Social Worker is pivotal in crafting your loved one’s care plan, basing it on the evaluation of living situation, disabilities, and family dynamics. For those with dementia, our Social Worker and aides will work together to develop routines and joint activities that the client and aide can enjoy together.

 

Every three months, the Social Worker visits your loved one’s home to ensure ongoing health and safety.

Elder care social services are at the crux of our mission and allow us to provide deeply personalized attention for your loved one.
